Your Medical Intake Form Is Essential to Safe & Accurate Prescribing

The medical intake form is one of the most important steps in beginning or continuing your GLP-1 treatment. The information you provide allows our licensed providers to evaluate your history, confirm your current or previous prescription, and determine the appropriate next steps.

Incomplete or inaccurate information may delay your prescription or shipment.


Providing Your Previous Prescription Information

If you are transferring from another provider or continuing treatment, you may upload:

  • A photo of your previous medication label
  • A copy of your prior prescription
  • Documentation that includes:
  • Your full name
  • The prescribing doctor’s name
  • The medication name
  • The dosage strength
  • The prescribed dose
  • The date issued

Providing this documentation helps our doctors verify your treatment history and avoid unnecessary delays.


Text & Email Reminders

To help streamline the process, we send:

  • Text (SMS) reminders to the phone number you provide
  • Email reminders to the address on file

These reminders prompt you to:

  • Complete your medical intake form fully and accurately
  • Review your prescription once issued

The 12-Hour Prescription Review Window

Once your provider finalizes your prescription, you will be notified and given a 12-hour window to review it.

During this time, you may:

  • Review the prescribed medication and dosing instructions
  • Request a re-evaluation if you believe changes are needed

If no re-evaluation is requested within 12 hours, the prescription will proceed to fulfillment and shipment.

To ensure the fastest delivery possible, medications are dispensed according to the provider’s prescription unless a re-evaluation request is submitted within that initial 12-hour period.

  1. Complete your intake form thoroughly
  2. Upload any prior prescription documentation
  3. Review your prescription promptly
  4. Respond quickly to any provider communications
